Retatrutide vs Survodutide: what is different?
Evidence checked 16 October 2026
These two peptides share two receptor targets: GLP-1 and glucagon. Retatrutide also targets a third receptor, GIP. (Retatrutide study, NEJM (2023)) (SYNCHRONIZE-1, NEJM (2026)) That makes them an interesting comparison: similar research ideas, but different molecules and different clinical programmes.
Here is the useful part: both now have large published human studies. (TRIUMPH-1, NEJM (2026)) (SYNCHRONIZE-1, NEJM (2026)) This guide explains the difference in everyday language, including why survodutide's liver research is a separate story from weight research.
The short answer
- Retatrutide: three receptor targets: GLP-1, glucagon and GIP. (Retatrutide study, NEJM (2023))
- Survodutide: two receptor targets: GLP-1 and glucagon. (SYNCHRONIZE-1, NEJM (2026))
- Why the comparison is interesting: the trials explore more than body weight, including sleep, pain or liver-related measurements in specific groups. (TRIUMPH-1, NEJM (2026)) (SYNCHRONIZE-MASLD, Nature Medicine (2026))
At a glance
The basic design
- Retatrutide
- One molecule designed to activate three receptor types.
- Survodutide
- One molecule designed to activate two receptor types.
(Retatrutide study, NEJM (2023)) (SYNCHRONIZE-1, NEJM (2026))
Receptor targets
- Retatrutide
- GLP-1, glucagon and GIP.
- Survodutide
- GLP-1 and glucagon.
(Retatrutide study, NEJM (2023)) (SYNCHRONIZE-1, NEJM (2026))
A large human study
- Retatrutide
- TRIUMPH-1: 2,339 adults studied over 80 weeks.
- Survodutide
- SYNCHRONIZE-1: 725 adults studied over 76 weeks.
Other questions being tested
- Retatrutide
- TRIUMPH-1 also measured knee pain and sleep-apnoea outcomes in relevant participant groups.
- Survodutide
- Separate trials have examined liver fat and liver disease.
(TRIUMPH-1, NEJM (2026)) (SYNCHRONIZE-MASLD, Nature Medicine (2026)) (Survodutide liver trial, NEJM (2024))
Two targets or three: what does that mean?
A receptor is a receiving point for a biological signal. An agonist is something that activates it. “Dual” means two receptor types; “triple” means three. (SYNCHRONIZE-1, NEJM (2026)) (Retatrutide study, NEJM (2023))
In this comparison, both molecules act at GLP-1 and glucagon receptors, while retatrutide also acts at GIP receptors. (Retatrutide study, NEJM (2023)) (SYNCHRONIZE-1, NEJM (2026)) That describes the targets, not an automatic ranking of which compound is better. The human studies are where the research questions are tested.
Three studies worth knowing
Retatrutide: TRIUMPH-1
Published in 2026, this trial involved 2,339 adults with obesity and without diabetes over 80 weeks. It compared retatrutide with a placebo, or dummy treatment. Eli Lilly funded the study. (TRIUMPH-1, NEJM (2026))
The headline finding: greater weight reduction than placebo, with improvements in knee pain and sleep-apnoea measurements in the participant groups studied for those conditions. (TRIUMPH-1, NEJM (2026))
Survodutide: SYNCHRONIZE-1
Published in 2026, this trial involved 725 adults without diabetes over 76 weeks. It compared survodutide with placebo, alongside lifestyle counselling. Boehringer Ingelheim funded the study. (SYNCHRONIZE-1, NEJM (2026))
The headline finding: greater weight reduction than placebo. (SYNCHRONIZE-1, NEJM (2026)) That is evidence from a controlled human trial, rather than a prediction based only on how the molecule is designed.
Survodutide: SYNCHRONIZE-MASLD
This separate 2026 study involved 216 adults with obesity and at-risk fatty liver disease over 48 weeks. Researchers measured liver fat with MRI scans as well as changes in body weight. (SYNCHRONIZE-MASLD, Nature Medicine (2026))
The headline finding: improvements in the study's liver-fat and weight outcomes compared with placebo. (SYNCHRONIZE-MASLD, Nature Medicine (2026)) Liver fat is the key distinction here: this result should not be read as proof that liver scarring was reversed.
The liver research, without the jargon
Three things are worth separating: fat in the liver, inflammation and scarring. MASH is a form of fatty liver disease with inflammation and liver damage; fibrosis means scarring. (NIDDK: liver fat, inflammation and scarring)
Survodutide has been studied in each of these research areas. A 2024 trial examined liver tissue in people with MASH and scarring, while the 2026 SYNCHRONIZE-MASLD trial measured liver fat by MRI. (Survodutide liver trial, NEJM (2024)) (SYNCHRONIZE-MASLD, Nature Medicine (2026)) These are different measurements. A finding about liver fat alone is not proof that scarring has reversed.
That is what makes this research worth following: the questions are becoming more specific, rather than stopping at a single weight measurement. (SYNCHRONIZE-MASLD, Nature Medicine (2026)) (Survodutide liver trial, NEJM (2024))
How to read the comparison fairly
These studies did not test retatrutide directly against survodutide. They used separate groups and different study lengths. (TRIUMPH-1, NEJM (2026)) (SYNCHRONIZE-1, NEJM (2026)) Their results are useful evidence for each molecule, not a simple head-to-head scorecard.
Safety findings belong in the picture too. Digestive side effects were common in the large trials. (TRIUMPH-1, NEJM (2026)) (SYNCHRONIZE-1, NEJM (2026)) The earlier retatrutide study also reported increases in heart rate. (Retatrutide study, NEJM (2023))
Both remain research-stage compounds rather than established personal-use options. Boehringer's October 2026 update describes survodutide as investigational and not approved for use; the TGA identifies retatrutide as an unapproved therapeutic good. (Boehringer development update, October 2026) (TGA research and approval context)
This guide explains published research, not recommendations for personal use. It provides no dosing, switching, combining or administration instructions. Trial findings do not establish the quality or medical suitability of a separately supplied research-labelled product.
